Real Choices for Community Living

You have a choice where and how you receive services you need. In June of 1999, the U.S. Supreme Court ruled in Olmstead vs. L.C., that "the unjustified institutionalization of people with disabilities is a form of discrimination under the Americans with Disabilities Act (ADA)."

As a direct result of the Supreme Court's decision, Home by Choice is assisting individuals in nursing homes to move out into the community into homes and apartments of their choice.

Who will benefit from these services?

A person with a disability who . . .

  • Currently lives in a nursing home or institution, and

  • Wants to move home with family members or into an affordable, accessible place in the community.

Services include:

  • DADS Community Care Services and other community resources

  • Grants for paying moving expenses, deposits, household items, and other related expenses.

  • Information on living in the community with suitable support.
